The Core Nutritional Recurring Delivery Program(RDP) is an optional agreement by which customers may participate in the program and place a continuing order to be charged against his/her credit card. By signing this agreement, customers desiring to participate in the RDP program agree to the following terms and conditions:
A. I will specify the quantity of products which I desire to receive every month on the attached Core Nutritional Recurring Delivery Enrollment Form which is part of this agreement.
B. I will supply Core Nutritional, in the space provided on RDP Enrollment Form, valid VISA,MASTERCARD,DISCOVER CARD or AMERICAN EXPRESS number(s) along with expiration date(s). All purchases made by me pursuant to the Recurring Delivery Program shall be paid by credit card.
C. I understand and agree that the products selected on the RDP Enrollment Form will be sent to me at the address listed each and every month as I have indicated unless I notify the Company in writing of changes I wish to make. The method(s) of payment I have selected will not be changed without my prior written notification. The Company will make the adjustments specified in my written notification no later than 30 days after receiving such notice.
D. I understand that the price of the specific products which I have chosen on my Enrollment Form may change due to reformulations, improvements or other reasons. When such price changes occur, the Company will notify me of any price increases and, unless I direct them to do otherwise, will continue to send me the products specified on my RDP Enrollment Form at the increased price. I understand I will receive a 100% refund on any product where the price has been increased if I notify the Company within 30 calendar days of the date of the product order, or the period specified by state law of the state where I reside for such refunds, whichever is longer.
E. I authorize the Company to establish an automatic credit card debit arrangement as specified in the RDP Enrollment Form to pay for each monthly Recurring Delivery order. The Company will make no other charges to my credit card account except those I have authorized.
F. I understand and agree that this Recurring Delivery Program Agreement may be immediately terminated without notice if credit card(s) to which product purchases have been charged become expired, canceled or otherwise terminated.
G. I understand and agree that this agreement will be suspended without notice if I violate any of the terms and conditions of the agreement. If this program agreement is terminated for any reason, I may reestablish a standing monthly order relationship with the Company buy submitting a new Enrollment Form.
H. I understand I must notify the company in writing to cancel RDP. If I do not notify the Company, the RDP order will continue to be shipped and my card debited every month.
I. I understand that I may cancel at any time. The Company will cancel the enrollment form within 30 calendar days after it receives such notification.
J. I understand that I will receive a 100% refund on any product ordered under the Program if I notify the Company within 30 calendar days of the date of the product order, or the specified by the law of the state where I reside for such refunds, whichever is longer.
You may cancel this transaction, without any penalty or obligation within three business days from the above date. If you cancel, any property traded in, any payments made by you under the contract or sale, and any negotiable instrument executed by you will be returned within 10 business days following receipt of our Notice of Cancellation by the seller, and any deposit interest arising out of the transaction will be canceled. If you cancel, you must return the goods to the seller in same condition as received unadulterated. If you agree to return the goods to the seller and fail to do so , then you remain liable for the performance of all obligations under the contract. To cancel this transaction, mail or deliver a signed copy of this Notice of cancellation or any other written notice, or send a telegram.
